We have been using PacsOne since December of 2005!
We have a 13 tb server and we will dangerously approach 100% storage space used by the end of 2018.
The images are neatly organized in yearly folders. I could easily just delete a couple year folders to regain some storage space.
My question is, what happens to the PacsOne database if I do this? Will it still show these studies during a search, but then show them as missing if I try to retrieve them? How do I eliminate this problem?

You should definitely NOT delete those Dicom image files externally without involving PacsOne Server, because doing so would result in discrepancies in the PacsOne Server database as the meta-records stored in the database would be linked to some non-existant files.
The correct way to make room for more storage space by removing some of the older studies (e.g., studies received more than 10 years ago), is from the "Tools->Automatic Purging Storage Directories" menu, and select either purge by the Study Date or Date When Studies Were Received, then enter the purge threshold (e.g., 10 years or 3650 days, etc). This way, when PacsOne purges the older studies, it will not only remove the corresponding raw Dicom image files, but also the database meta-records linked to those image files.
