Would it be possible to add dicom print options to print overlays like Patient Name, Patient ID, Exam Date and Patient Date of Birth?

DICOM Print Overlays
Patient Name, Patient ID, Exam Date, etc. can be printed as annotation boxes instead of overlay boxes (burned 1-bit pixel data), but neither the annotation box nor the image overlay box is required to be supported by the printer.

The Film Printer is an Agfa Drystar 5300 (DS5300). I found the following in the Dicom Conformance Statement. I could forward the complete pdf file if necessary.

According to the information you posted, this printer supports the annotation box sop class but not the overlay box class, so you'd not be able to print text boxes using the burned/overlay data but you can use the annotation boxes to fit the text strings, e.g., Patient Name, Exam Date, etc.
We'd need the full conformance statement of this printer since the annotation formats it offers are most likely proprietary (that's why the annotation box sop class is optional), in order to implement these annotation formats for the next version of PacsOne Server. We'd probably need some of your time as well to test the implementation because we don't have access to such a printer ourselves. If you're ok with this, please send the conformance statement to mailto:pacsone@pacsone.net.

Change Display Annotation boxes FONT SIZE BIGGER
Can we change the font size bigger when printed out at annotation display box 1 and box 2 ?

The font size of the annotation display box should be configurable from the Dicom printer, as PacsOne can set the annotation text and position, but not the font size. Check the user manual or technical support for your Dicom printer, and see how to set the font size for the annotation display boxes.
